I am too much impressed on the preparation of the histopathology course. HP photo, it helped me in diagnosing skin biopsies in Hansan's disease, nice. I want to purchase one copy of the book with colour photos, can you send it, I will pay. Tell me any other course. Some time ago in July, just before departure for Zambia I received from you a CD with the AIFO-training as published on the AIFO website. I promised some feedback, which I send to you just prior to my next visit to Zambia (September 05). I used the CD during a training to provincial and district leprosy workers, mostly involved in both program as well as patient management. The course aimed to be a restart of a program which had come to a virtual stand-still after health sector reforms. So the training objectives were related to basic knowledge and skills. Training tools included a computer connected to a beamer and part of the time a stand alone desktop was available to students to practice. Only limited time was spent behind the monitor with real studying as priority had to be given to practical exercises (doing and repeating the physical examination(s) and to exercising basic computer skills. However, the students were very enthusiastic and impressed by the leprosy information so beautifully within reach. Therefore I think that introduction to basic computer skills should be part of the curriculum if not available. We also used the interactive CD from the Welcome Trust on Leprosy, a very good CD which, unfortunately, does not run as well on Windows 200x XP as it does on Windows 95/98!

-In the field an internet connection is often not available; the course should be available as well on a CD and include fool-proof instructions on installation.

-The CD should be tested in advance on the general configuration. The slides/screens CD you send me did not always fit well on the screen and therefore a few clicks were needed all the time to adjust it and to reach the navigation buttons. I of course realized that you send me the CD in fact when you were not yet completely finished with it. You were also pressed by my hurry up request!

-The contents of the CD were easily recognizable as it was for a great part, based on ILEP technical guides. I experienced this as a great advantage. In my opinion, students should know their way in their books, notes, and also CDs. Only then they will consult their sources during their work. Therefore I trained with them also the skills on accessing information (rather then always putting accent only on knowledge).

During the next trip to Zambia, which is a follow-up of the July-training, I will carry the CD with me again.

I think these courses are very clear and well-presented.  We should provide links.

There are two minor points that I would raise, concerning the correct linkage between text and picture:

In the course on Diagnosis and Treatment, slide 8 is the start of a section on testing for loss of sensation in the skin in order to diagnose leprosy.  Subsequent pictures are correct, but slide 8 is actually a picture of someone being tested with a ball-point for sensory loss in the palm of the hand.  There is quite a lot of confusion about this apparently, but it should be fairly simple – for diagnosis, one is testing for sensation in skin patches only, using a very sensitive tool such as a wisp of cotton wool (some later pictures show this well).  In a known cases of leprosy (i.e. when the diagnosis has been established), we are interested in nerve function, which can deteriorate during and after treatment – in these cases, we test nerve function every month in various ways, including the use of monofilaments (or a ball-point pen) to test for loss of sensation in the hands and feet.  Thus in slide 8 the text is correct, but the picture is not right – it should show a skin patch being tested.

In the course on Self-care for POD, there is not much to quibble about, as the overall message is very good.  However, I noted that slide 16 is talking about self-care of hands to prevent wounds and stiffness, while the picture is actually a health worker doing a voluntary muscle test (VMT), to test the integrity of the ulnar nerve.

I’m not an expert in histopathology, so cannot comment on the detail, but this course seems to be clearly presented and the authors are well-known experts.
