Dear Salvatore,

 

I am looking for examples of a (electronic available) leprosy (clinical) patient card with details of the dermatological and neurological evaluation, a representation of the human body in which the lesions can be drawn, etc.  With small boxes or field which can be ticked.  For example: a box for 1 lesion, one for 2 - 5 lesions, one for 6 and more lesions, and may be one to note down the actual number of lesions. Boxes for classification like I, TT, BT, BB, BL, LL; etc.

 

Such a card seems not to be very popular anymore in these times of decentralisation, but I still think them very useful and more so for doctors who see only a few patients a year.  A patient card well drawn, following the flow of examination step by step, reminding the one with less experience what to do next, and at the same time leaving a valuable record of the patient examination at the start of treatment.  As was discussed recently, the appearance of lesions can change rapidly and sometimes lesions can disappear altogether in a relatively short time.
