Dear Dr Noto,

 

We are living in serious times but a well developed sense of humour is still appreciated.  Wolfgang's comments, "Kathmandu recommendations" are apposite and should be taken seriously otherwise many early cases will be neglected until disability ensues.  Surely this is NOT a consequence any of us would welcome.
