Ref.:     Zafirlukast

From:   Ryan T. Oxford, UK

Dear Salvatore,

In reply to Dr Ganapati (LML May 30th, 2007).

It may be worth pointing out that the commonest reason for stopping this leukotriene antagonist is ineffectiveness.  It is safe, except that there is a continuing discussion whether it can convert one kind of inflammatory response into another more vasculitic (Churg Strauss).  Reminding me of a long term discussion whether suppressing neutrophil mechanisms to eliminate immune complexes or bacteria results in slower elimination of noxious agents which is their job.  It dates from the early days of prescribing steroids for Henoch Schoenlein purpura and whether it protected the kidney or not.  I would make certain of close follow up of any kind of immune complex disease.

Here is the Pub Med item for example which says its safe but not very effective.  Mostly used for asthma but also Rheumatoid arthritis and inflammatory bowel disease.  Dermatologists use it for Urticaria without much success.
