information about treatment of type one reaction (reversal reaction)
in HIV positive patients.

Dear Dr Noto,
I work in Pokhara, Nepal, at the Green
pastures hospital as senior medical officer. I have a newly diagnosed,
smear +ve leprosy patient and, he is also HIV positive. He is 43 years old and has
type one (reversal) reaction and left foot drop with duration of 3
months.
I have just started first dose of
multibacillary multi drug therapy (MB MDT). He is also on antiretroviral (ARV)
therapy.
My main concern is for the
management of reversal reaction and nerve impairment. I DO NOT
HAVE ENOUGH EXPERIENCE OF PREDNISOLONE ON SUCH PATIENTS. I would like to get
information regarding such problem.
Can we use prednisolone? Or
any other means of treatment? I
hope some colleagues can help me and share their experience.
