Ref.:     Alternative treatment regimens to standard W.H.O. multidrug therapy (MDT)

Dear Salvatore,


I would like to request advise about alternative treatment regimens to MDT.  I have two patients, here in the Dominican Republic who do not tolerate dapsone.

 

The first one is a young female with tuberculoid leprosy.  She develops metahaemoglobinenemia, I think, her lips, fingers and toes become purple as soon as she takes dapsone.  The second one is a multibacillary (MB) leprosy case who suffers from hepatitis C and her gastroenterologist did not allowed the use of rifampin or dapsone.

I think both could be treated with rifampicin, ofloxacine and minocicline (ROM) but there is not minocicline available in this country.

I would be grateful to receive advises on what could be done in such cases?
