Concerning Dr
Kamaladasa's case (LML Oct. 18th, 2008), I cannot explain
the methaemoglobinaemia but the signs at the right diaphragm and the
general clinical picture suggest the possibility of a sub-diaphragmatic abscess,
such as an amoebic liver abscess. An
ultrasound scan and amoebic serology might be helpful. Steroids might aggravate the situation and
mask its seriousness.
